The card can ONLY BE USED AT BIOBALANCE HEALTH FOR CONSULTATIONS AND/OR PELLET INSERTIONS.

Gift card must be presented at time of use - printed or on phone.

The card cannot be used to pay for prescription medications, supplements, or blood work.

Gift cards are not refundable and cannot be exchanged for cash value. Please disregard the instructions for redemption in the gift certificate email. They can only be redeemed in-office and not online. At the moment, our system is not able to remove those directions.

All cards expire 1 year from date of purchase.
